      Hi,
      yes, you need to submit attested document. There will be two copy, one is attested by gazetted officer and one by self attested.

      Yes, If your name is not mention at birth certificate then you need to submit an affidavit.

It is established at all ILP center as well as base center for you. So, you don't need to take any books or notes from home with you.
